Bug fixes for the file changes I made. With the addition of the mode
flags, allowing file appending, and these changes, I can code generate
std_logic_1164 again and simulate a simple example.

I had to comment out a couple of lines in
IIRScram_FileDeclaration::_publish_cc related to file declarations in
package scope. I've commented them out with an explanation. Hopefully
DJ can look when he is back.
