Some minor formatting cleanups.

When we publish EnumerationLiterals, we also need to #include the header
file.  This was causing a fair number of build issues in Ashenden.

Working on fixing code generation for "addChild" for more cases.  This 
should fix some of the publish_cc errors that we're seeing and turn them
into build errors for the moment.
